UART mux device name template

Device name template for the UART mux Devices. First device would
have name $(UART_MUX_DEVICE_NAME)_0, etc. User will access muxed
UART using this name.

UART_MUX && CONSOLE

(Includes any dependencies from ifs and menus.)
